Trosolwg o'r swydd
Our Acute Medicine Department has an excellent reputation following on from its pioneering days of developing the concept of Ambulatory Care in the early 2000s, with Ambulatory Emergency Care being an ongoing strength of the Department. We are the home unit for the ‘Amb score’, a sensitive scoring system derived to select out potential ambulatory emergency care patients from the unselected medical take. Our experienced and extremely dedicated team of nursing staff in the Ambulatory Emergency Care Unit (AECU) continue to use this during their patient triage, to ensure we operate a ‘pull’ system into AECU and facilitate our high same day discharge rates, overseeing the care of around 35-40 patients per day. Recognising the value and growing patient turnover of AECU, we moved into a new unit in July 2020, which includes a spacious waiting room, 2 private triage rooms, 4 consultation rooms, a trolley bay with 6 trolleys and chairs, and have a dedicated ultrasound machine with echocardiography capabilities. We have great links with our on-site Radiology Department, which hosts the ‘Diagnostic Hub’ for the UHB, with access to same day ultrasound and CT scans routinely, and MR imaging also when the need arises.

The Acute Medicine Department consists of 9 Consultants, who come with a variety of clinical and non-clinical interests. We are supported clinically in our AEC and inpatient AMU work by a vital trio of: an SAS Doctor, Physician Associate and Advanced Nurse Practitioner, alongside doctors in training. We are joined by 1-2 Acute Medicine Higher Specialty Trainees per year, and doctors in General Practice Vocational Training Schemes and Foundation Programme Training rotate through the department every few months. The resident component of the Consultant on call shifts are with the medical on call team, usually consisting of 1 FP1/2, 2 IMT stage 1 (1 on ward cover, and 1 on the take), 1 GPVTS twilight shift cover, and an IM4+ (or equivalent). During the non-resident period of the on call there is 1 FP1/2, 1 IMT stage 1 (or equivalent) and an IM4+ (or equivalent) doctor on site.

Clinical
Acute Physicians in the Royal Glamorgan Hospital have weekday responsibilities covering 3 acute areas of work. There is one Acute Physician allocated to each of the following duties each weekday:
- Acute Medicine Unit (AMU) – daily Consultant ward round, accompanied by junior clinical staff, each morning of the Acute Medicine patients within AMU and the Emergency Department
- Medicine on call – reviewing new patients on the post-take ward round between 08:00 – 21:00 on weekdays, and providing non-resident cover for the medical on call team between 21:00 – 08:00 Monday to Saturday morning
- Ambulatory Emergency Care Unit (AECU) – working with junior clinical staff and our team of experienced nursing staff to review new patients within AECU. Within this role there is also an expectation that the designated Acute Physician will provide advice to requesting Primary Care colleagues, Paramedics, and other Specialty colleagues regarding the ongoing care of patients with suspected acute medical problems. Acute Physicians are also welcome use AECU to review patients they discharge under an ‘early supported discharge’ plan.
All Acute Physicians within the Royal Glamorgan Hospital run formal outpatient clinics to provide longer term follow up for their patients.
Out of Hours Commitments
Acute Physicians provide cover for the Medical On Call on a resident basis Monday – Friday (inclusive) 08:00 – 21:00, and on a non-resident basis 21:00 – 08:00. This is with a frequency of around 1 in 7 weekdays. The period from Saturday 08:00 – Monday 08:00 is covered by General Medicine colleagues, so there is no requirement for weekend working after 08:00 on Saturdays for the successful Acute Physician candidate.
