Job overview

Job Purpose:

  • Provide technical support to the clinical pharmacist and other pharmacy services within general practice and the Primary Care Network (PCN) to ensure professional operational standards and excellence in pharmaceutical care and safety are maintained
  • Ensure delivery of safe, effective and efficient systems for medicines related clinical correspondence, repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes

Main duties of the job

  • Undertake patient facing and patient supporting roles to ensure effective medicines use, through shared-decision making conversations with patient
  • Carry out medicines optimisation tasks including effective medicine administration (e.g. checking inhaler technique), supporting medication reviews, and medicines reconciliation. Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively
  • Support, as determined by the PCN, medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings and linking with local community pharmacists
  • Provide specialist expertise, where competent, to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities
  • Take a central role in the clinical aspects of shared care protocols and liaising with specialist pharmacists for more complex patients
  • Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing
  •  Assist in the delivery of medicines optimisation and management incentive schemes and patient safety audits

Detailed job description and main responsibilities

  • Alert pharmacy technician Coach to any issues compromising the quality of projects and operational workstreams
  •  Effectively manage own time, workload and resources
  • To support training and development of administrative staff to support pharmacy service in line with protocols and procedures
  • Work within risk management and health and safety policies and procedures within GP practices and other providers
  • Adhere to a robust process for recording learning events, complaints, accidents and significant events
  • To provide support to assist with the development and management of any changes or new processes and systems that need implementing.
  • To carry out any other appropriate duties as determined by the organisation
  • Should hold GPhC registration

  • Portfolio of evidence of continuous professional development and post qualifying experience

  • Membership with Royal Pharmaceutical Society

  • Medicines Reconciliation Certificate or equivalent

  • Understanding of professional ethics and application in practice. Experience of prioritising work load and meeting deadlines. Previous experience working within a patient facing role

Experience - Desirable

  • Experience of prescribing/ clinical systems such as SystmOne and EMIS Web. Recent previous experience with comparable role in a General Practice. Able to undertake audit/quality improvement methodology

  • Good communication skills . Ability to recognise and seize opportunities for change and development within the service. Behaviour change and motivational interview skills.
