Specialist Clinical Pharmacist: Medical Admissions

We are delighted to invite applications to join our team of specialist clinical pharmacists on the Medical Admissions Unit at the Queens Medical Centre.

This exciting, fixed term/secondment opportunity covering maternity leave has arisen for an experienced clinical pharmacist looking for a new opportunity to develop their clinical, leadership and management skills.

We are looking for a motivated, forward-thinking, dynamic individual to join our senior team, who is keen to learn, innovate and push the service forward.

The admissions unit is one of the largest in the country, so we have built a large clinical pharmacy team to deliver an excellent 7-day service.

The post offers an outstanding opportunity to develop your portfolio of specialist knowledge and skills for the management of patients as part of the friendly, progressive and dynamic multidisciplinary team, as well as build on your leadership abilities.

Prif ddyletswyddau'r swydd

You will be expected to be part of the leadership team for acute medicine, which currently consists of 6 specialist pharmacist posts, and take on a specific element of the service delivery e.g. education, training, governance, operational etc.

You will take on a specialty link role which will ensure specialty specific knowledge is maintained & up to date within the team.

You will work closely with the MDT, on & off the ward to support governance, education and medication safety, particularly supporting medicine optimisation on ward rounds, utilising your prescribing skills.

You will be expected to line manage one or more of the band 7 pharmacists on the team, providing mentoring, and supporting them with their objectives which may include starting out as an independent prescriber.

You will be responsible for ensuring the provision of an excellent clinical pharmacy service to the acute medicine specialty.

Leading on an aspect of the clinical pharmacy service to Acute Medicine

Actively participating in consultant-led ward rounds to promote and support medicines optimisation, antimicrobial stewardship and effective discharge planning by utilising prescribing skills

Providing an advanced level of clinical pharmacy service to the specialty

Involvement in education and training of a variety of healthcare professionals including diploma & pre-registration pharmacists, undergraduates and trainee advanced nurse practitioners

Whilst this post is commissioned to be full time (equivalent of 37.5 hours per week) we would welcome applications from those wanting a better work life balance. Training and Qualifications

  • Diploma/MSc in clinical pharmacy or equivalent qualification or experience
  • Qualified independent prescriber
  • B.Pharm/BSc (Pharmacy) or MPharm

  • Related postgraduate qualifications
  • Play an active role in a relevant national network/specialist pharmacist group

Experience

  • Broad previous experience (post-registration) of clinical pharmacy, particularly in acute settings
  • Broad knowledge base of a range of clinical areas
  • Experience of developing and implementing medicines and service related policies for a specialty
  • Experience of training and education of both pharmacy staff and other healthcare professionals
  • Experience of provision of specialist clinical pharmacy review and advice relating to the safe use of medicines
  • Some experience of line management, including effective performance management, recruitment and allocation of staff
  • Some experience of excellent leadership and management skills

  • Experience in undertaking, supervising and reporting/presenting audit / research work at local (Trust) level
  • Experience of working actively on national groups
  • Experience in project management (including department-wide roles) and evidence of completed outcomes

  • Empathy with patients, relatives and other pharmacy service users
  • Proven ability to influence and negotiate with relevant senior clinicians, managers, stakeholders

  • Able to defend opinions in a confident and constructive manner when challenged

  • Evidence of writing compelling business cases or case of need for service development
  • Teaching skills

Analytical and Judgement

  • Experience of assessing and minimising risk associated with service provision and within medication processes
  • Critical evaluation of literature and literature searching skills for analysing, interpreting medicines information, production issues, drug therapy decisions and patient information
  • Ability to work alone and make decisions within agreed parameters ; and appropriate referral / escalation as necessary
  • Think creatively to support departmental strategy and overall Trust objectives, with evidence of incorporating innovative and 'best practice' into current practice

  • Ability to be flexible and manage change in a complex and changing healthcare environment
