Early Support Discharge Rehabilitation Support Worker

We are looking for a bright enthusiastic and motivated individual to join our Hillingdon Stroke ESD Team. The main role is to provide rehabilitation programmes for patients in the patient’s place of residence under the guidance of therapists for patients who have had a recent stroke. This post would be an exciting opportunity for all rehab/therapy assistants who are looking to further develop their skills and expertise in the area of stroke rehabilitation. The post holder must have very good communication skills and be able to work as team player.

The position will require you to visit various locations throughout our supported area, it is essential that candidates hold a driver’s license and have use of a car for business purposes.

There is a possibility of the successful candidate working across CNWL as part of the rotation. The post holder will be expected to take part in seven day working.

The successful applicant may have contact with patients or service users. As an NHS Trust we strongly encourage and support vaccination as this remains the best way to protect yourself, your family, your colleagues and of course patients and service users when working on our healthcare settings.

  • To work as rehabilitation support worker within the Hillingdon Stroke ESD team delivering care to people with acute stroke/neurological conditions in their own homes and/or designated buildings. 
  • To assist the therapists (OT/PT/SLT) by carrying out delegated aspects of patient care without direct supervision, reporting back patient progress and informing qualified staff of any issues. There is a possibility of successful candidate to work across Community Adult Rehabilitation Service.
  • For a defined and delegated group of patients-to undertake the assessment using investigative and analytical skills. To use clinical reasoning and utilising assessment and treatment skills to formulate treatment programmes within protocols and scope of practice.
  • To undertake specific skilled support work to aid the rehabilitation of patients.
  • To undertake seven day working

To be responsible and accountable for all aspects of your own work for the patients in your care.

To undertake a range of assessments and therapy interventions with patients once deemed competent by therapists within the team.

To use clinical assessments and reasoning skills to develop, deliver and evaluate individual treatment programmes based on a sound knowledge of evidence based practice and treatment options with the support of therapists.

Assess patient understanding of treatment proposals, gain valid informed consent, and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.

To support, advise and educate family members in order that they are best able to support patient’s rehabilitation if required.

Use a range of verbal and non-verbal communication tools to communicate effectively with patients to progress rehabilitation and treatment programmes. This will include patients who may have difficulties in understanding or communicating.

To empower patients to participate in their rehabilitation and where able self-manage their condition.

To manage clinical risk within own caseload under direct clinical supervision.

To be responsible for the safe and competent use of all appropriate equipment.

To provide support and education to patients and carers regarding aspects of the service, e.g. equipment fitting.

Be aware of referral procedures and make appropriate referrals to other agencies and services in accordance with patients and/or family needs.

Qualifications

  • Education to GCSE level or equivalent life skills acquired through experience
  • Full & current UK driving licence and access to a vehicle for work purposes
  • Willingness to undertake other relevant training

Experience

  • Experience of working within a multi disciplinary team
  • Experience in stroke rehabilitation/ previous neuro experience
  • Relevant experience as a therapy assistant in a related field

  • Experience of working with patients with physical/psychological needs
  • Experience of working with people who have communication disorders / difficulties
  • NHS Experience

Knowledge & Skills

  • An understanding of the needs and difficulties of people with physical, cognitive, sensory and/or mental health issues
  • Ability to teach skills to patients
  • Ability to organise self and work under pressure
  • Health & safety and risk awareness
  • Good written/verbal communication skills
  • Confident in IT skills, Microsoft Office and patient data recording systems
