Non Admitted Transformation Clinical Lead (Planned care/APC)

Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.

This post is designed to provide Primary Care clinical support and leadership for the transformation of non-admitted pathways across SEL. The postholder will work with Acute Provider Collaborative (APC) Networks to drive this agenda. They will work closely with APC colleagues and the Integrated Care Board (ICB) Planned Care team as well as other specialty and borough level clinical leads working in this area.

The postholder will play a key role in driving the transformation agenda and be an advocate for innovation and improvement. Working closely with the Network Clinical and programme Leads they will ensure that a clinical voice is heard and represented in the development, design and implementation of initiatives and new models of care linking with colleagues across the healthcare system to maintain input and engagement. They will also emphasise the inclusion of patient perspectives in the development of care models, ensuring that the patient voice is not only heard but is a guiding force in the transformation process. They will have strong interpersonal and relationship management skills, and will be able to work effectively with clinical and managerial colleagues across the system.

The postholder will support in the development and implementation of initiatives such as:

•    Expanding and developing advice and guidance and clinical triage 
•    Development of clinical guidelines 
•    Rolling out of patient-initiated follow-up 
•    Supporting patients whilst they are waiting 
•    Exploring opportunities for expanding and enhancing community provision
•    Ensuring that every outpatient appointment is value adding 

It is not essential that you have experience of leadership at system level. We will be providing development support for all of those with the skills and appetite to step into leadership roles with SEL and are keen to ensure a more diverse range of clinical and care professionals able to participate in system leadership roles – better reflecting our ambitions around integration and the multidisciplinary nature of care, as well as the diversity of the communities we serve

Qualifications

  • Qualified care and/or clinical professional i.e. NMC, HCPC, social work register, GMC etc. Educated to Masters level or equivalent level of experience of working at a senior level in specialist area.

Experience

  • Experience of transformational/innovative service delivery, productivity and/or transformation policy implementation.
  • Experience of Engagement of patients and public and the principles of co-production in service design and delivery

  • Experience of Setting strategic direction in consultation and partnership with stakeholders
  • Cross organisational and/or professional boundary collaboration experience
  • Previous experience of being part of peer networks and collaborative working
