Are you looking for an exciting change within the pharmacy world? Perhaps a move from secondary care but still wanting to work as a clinical pharmacist and help patients' with their medicines? If yes, this could be the post for you:
Central London Community Healthcare NHS Trust are looking to recruit a dynamic and innovative pharmacist to join the Medicines Management Team.
Key components of the post include working within the frailty Hospital @ Home service which covers the Barnet borough, ensuring an integrated approach to the management of medicines in high risk patients in order to prevent avoidable hospital admissions and facilitating early discharges. This will involve working as part of the multi-disciplinary team, managing your own caseload and conducting clinical medication reviews in patients' homes. This role will offer flexibility with your working day being based with the Hospital @ Home team and also the ability for remote working where appropriate.
The pharmacist will act as a source of pharmaceutical expertise to various health and social care professionals e.g. GPs, nurses, therapists, hospital clinicians, carers, families, community pharmacists and care coordinators. Continuous evaluation of the service will also be key to demonstrate value and areas for further development.

The post holder will predominantly be working in Barnet and the role will involve lone working and travelling between sites. The Medicines Management team is based at Parsons Green. The post holder may also be required to travel to other sites and services within the organisation so will require flexibility as the organisation continues to grow (please check the CLCH website to see the geography covered by CLCH). A car would be useful for travelling in outer London or outside of London but is not essential.
The post holder must have significant post-registration experience in hospital pharmacy (specifically care of the elderly/frailty) and a postgraduate qualification to at least diploma level. Previous experience in community health services or primary care is desirable. Sound knowledge of prescribing and medicines management issues faced in both primary and secondary care is essential with excellent decision-making and problem solving skills. Additionally, compassion, flexibility, excellent communication and time management skills and the ability to build relationships with various health and social care professionals e.g. GPs, hospital clinicians, district nurses, carers, family, community pharmacists and care coordinators.

• To deliver a high-quality Medicines Optimisation Service to improve medicines-related outcomes for high-risk patients within the frailty Hospital @ Home caseload and ensure the safe, cost-effective, and evidence-based use of medicines
• Work closely with the other Hospital @ Home pharmacists across the trust to support each other and ensure standards and goals are transcended across services.
• To build relationships with key stakeholders that may use or refer into the service. This includes proactively engaging with the medical teams within surrounding hospitals, clinics, PCNs, GPs etc.
• Conduct clinical medication reviews with patients in their own homes
• Ensure proactive liaison with relevant healthcare professionals regarding changes to medication regimes. e.g. regular GP, nursing/ care home, community pharmacist ensuring they have the information they need and that follow-up appointments are clear where necessary

- Professional registration with the General Pharmaceutical Council (GPhC)
