Bexley Primary Secondary Care Interface Clinical Lead

CCPL Tier 2

Trosolwg o'r swydd

Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.

The interface between primary and secondary is one of the key channels through which patients and their care flow. It needs to be patient focussed, delivering the best possible experience of care and clinical outcomes whilst ensuring that there is efficient use of resources. However, frustrations are common with all stakeholders having to contend with multiple touch points, at times inadequate communication, failure to make use of all the expertise in the system at the right time and poor use of patients’ and clinicians’ time. The interface is complex and relies on processes which may not be fit for purpose, incompatible IT systems and, often, poor transfer of information.

The ICB is seeking a clinical and care professional lead to support the Bexley Place system to address the 4 specific interface domains detailed within the NHS England Delivery plan for recovering access to primary care. These include: onward referrals, complete care, call and recall and clear points of contact.

The postholder will develop and expand ‘Place’ or ‘Trust-based’ interface groups to define, prioritise and address locally identified issues with a focus on building relationships across the interface to address the problems and define effective and sustainable solutions.

Gweithio i'n sefydliad

From the Catford Cat to the Brixton Windmill and Chislehurst Caves, South East London is a vibrant, fun, unique and richly diverse place to live, grow, learn and work.

The South East London Integrated Care System (SELICS) brings together the organisations responsible for health and care services in south east London, making the greatest possible contribution to the health and wellbeing of people living in our six boroughs.

SELICS is made up of staff working in many different roles, in different settings, employed in different ways, by a wide range of partner organisations, and are at the centre of our collective ambition for greater integration and better care. Employing organisations include NHS Trusts, GP practices, Social Care and Voluntary sector organisations, social enterprises, dental surgeries, pharmacies and optometrists.
We have an ambitious People Strategy that brings these partners together to deliver the ‘one workforce’ ambition, making the health and care system a better place to work and live.
