North Bexley Clinical Care Professional Lead for LCN Lead

Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.
As part of the clinical and care professional leadership programme for Bexley we are looking for a Local Care Network (LCN) Clinical Lead who can co-lead the development of integrated neighbourhood team for North Bexley. This role will be instrumental in better joining up primary care with the wider health and social care integration that is already well established through Bexley Care. This leader will be responsible for driving forward the Fuller Report for this geography in Bexley, leading system wide change, on the ground that seeks to address service gaps, better align collective resources to need and can deliver a model where professionals identify with a geography not an organisation.
We are therefore seeking a clinical professional who is currently working in a front-line, patient facing role in the Bexley Healthcare System who is dedicated to taking on this leadership role 2 session per week on a fixed term contract of employment or secondment. This individual needs to be passionate about improving patient care through working collaboratively with other service provider partners to make the strategic ambitions of more integrated care, an operational reality in Bexley.

This role will need to drive forward change across the North Bexley Local Care Network to achieve the following:
• Make “Place-based” priorities a reality on the ground through strengthened multi-disciplinary teams and links with community-based organisations
• Accelerate the development of PCNs to work beyond general practice and enhance the use of ARRS resource for at scale delivery models
• Provide capacity to develop relationships with voluntary, community and acute professionals for the benefit of patient care
• Development of new ways of working at a LCN level to address identified priorities
• Focus on addressing the stark health inequalities that populations in the north of the borough face.

- Qualified care and/or clinical professional i.e. NMC, HCPC, social work register, GMC etc. Educated to Masters level or equivalent level of experience of working at a senior level in specialist area.

- An in-depth understanding of health and care provision and awareness of current ICS/system policies and priorities
