Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.

Mental health providers in Bexley are working together with commissioners to transform the way that community mental health services operate at place in line with the expectations of the NHS Long Term Plan and to better meet the needs of local people. Increasing levels of demand and complexity in our population, coupled with new funding flows for mental health mean we have an exciting opportunity to reimagine how we deliver care across all age groups and levels of need. Our aims include a greater focus on prevention and wellbeing and delivering more integrated and personalised models of care.

The aim is not just to redesign care pathways but to develop a new ethos and culture that helps transform how we work together with partners as ‘one team’ sharing care.

Prif ddyletswyddau'r swydd

We are seeking to recruit an experienced clinical/care professional to work closely with commissioners and operational teams to provide an independent clinical/care leadership perspective and help define, lead and drive this change. The role represents a unique opportunity to help shape and deliver an ambitious and radical programme of change, co-produced with colleagues from across the health and care landscape and with service users - for the benefit of our local populations.

Immediate areas of focus will include:
•    Implementation of an integrated Mental Health and Wellbeing ‘hub’, bringing together Oxleas, Mind in Bexley and other agencies in a co-located space to facilitate holistic and person-centred triage of referrals, brief interventions and personalised care and support, including enhanced opportunities for personal health budgets
•    Design and development of new integrated care pathways for a range of conditions across primary, secondary, and tertiary services
•    Substantially increasing delivery and uptake of annual physical health checks for adults with Severe Mental Illness (SMI) in line with nationally mandated targets and ensuring meaningful interventions to improve health outcomes at both individual and population level to reduce significant health inequalities
•    Supporting commissioners with decision making in respect of individual complex and high-cost funding cases
•    Improving effective transition between children’s and adult services

  • Qualified care and/or clinical professional i.e. NMC, HCPC, social work register, GMC etc. Educated to Masters level or equivalent level of experience of working at a senior level in specialist area.

  • An in-depth understanding of health and care provision and awareness of current ICS/system policies and priorities
