Job overview

An opportunity has arisen for an individual to join the Community Integrated Learning Disability Team.   The Post holder will work as part of the Joint Health and Social Services Community 

Learning Disability Team.

We are looking for enthusiastic and motivated individuals who have excellent values, interpersonal and communication skills. You should be innovative, person centred and committed to improving outcomes for people with a learning disability across health, social care and third sector services.

Main duties of the job

The team offers an integrated specialist health and social care service to people with a learning disability and their carers, living in community settings. You will carry a caseload and work as an allocated nurse to service users with complex health needs. You will have experience of completing health assessments and person centred health actions plans and will be able to facilitate and support ongoing access to healthcare.

Detailed job description and main responsibilities

To participate in the assessment, monitoring and evaluation of programme of care for people with a learning disability

To liaise effectively with primary care teams and all professional agencies in the development and planning of care for the person with a learning disability

To use specialist knowledge effectively, informing clients and carers about clinical aspects of learning disability

Attend and contribute to PCP's and joint assessment meetings by being the client's facilitator and providing verbal/written reports of the clients needs

To be personally accountable for professional and ethical action and ensure compliance with the NMC Code of Conduct and Trust policies and procedures.

Essential criteria
  • Nursing Qualification RNLD

Experience

Essential criteria
  • Experience of working as part of a multidisciplinary team and with partner agencies.
  • Experience of working in a community setting with adults with a learning disability.
  • Knowledge of the current legislative framework and good practice guidance regarding supporting people with Learning Disabilities.
  • Knowledge of the Mental Capacity Act.
  • Experience in the completion of health screenings, health assessments and the completion of care and intervention plans.
  • Experience in the development of Person Centered plans and Health Action Plans.
Desirable criteria
  • Experience in working collaboratively with colleagues in primary and secondary health settings, social care and third sector agencies.
  • Experience in the identification and delivery of training.
  • Experience in the use of medication to support people with a learning disability and challenging behaviour and/or mental ill health.
  • Experience in case/care management and the facilitation of multi-disciplinary planning meetings.
