This is an outstanding opportunity to join our team at the Acute Medicine Department at Royal Blackburn Teaching Hospital. The acute medical unit (AMU) in Blackburn was established in 1993 and has been the longest continuously operating AMU in the country. The Acute Medicine Department currently consists of a 73-bedded Acute Medical Unit (AMU), an Ambulatory Emergency Care Unit (AECU) with 10 assessment cubicles, and a Short Stay Ward with 23 beds. Our most recent expansion in 2020 was a purpose-built modern AMU-B with continuous cardiac monitoring in high observation areas and point-of-care ultrasound. Our AECU provides an outstanding service in same-day ambulatory emergency care. Together with Hospital at Home sessions, we provide acute medical care to a significant number of non-hospitalised patients. The unit has also a dedicated outreach service into the emergency department. Our weekly departmental teaching programme and twice-weekly radiology teaching sessions are highly valued within the team. The department has actively embraced the use of point-of-care ultrasound in acute medicine, and we have become an accredited centre for delivering the FAMUS course. Out-of-hours consultant cover is provided by the on-call medical consultant. Weekend duties are long-day shifts Saturday and Sunday on AECU every 6 weeks. The appointment is full-time and there will be a requirement for flexible hours in accordance with service needs as they develop.

- Hands-on involvement with medical admissions ensuring the timely development of appropriate management plans for patients.
- Employing alternatives to in-patient management where clinically appropriate.
- Supervision and co-ordination of junior doctors within the unit.
- Helping to maintain multi-professional systems of care for common conditions.
- Teaching and training of medical and nursing staff, both informal and formal.
- Adherence to the clinical governance framework.

Clinical sessions are currently provided in AMU, AECU and Fast Flow Ward.
Individual allocation of shifts in certain areas will depend on session availability and individual preferences.
Available shifts on AMU, AECU, AMU and Fast Flow Ward are as follows (Monday to Friday):
- AMU 08:00-12:00 (1 PA): 1 shift per day
- AMU 12:00-16:30 (1.1 PA): 1 shift per day
- AMU 16:30-20:30 (1.1 PA): 1 shift per day
- AECU 10:00-14:00 (1 PA): 1 shift per day
- AECU 14:00-19:00 (1.25 PA): 2 shift per day
- AECU 19:00-22:00 (1 PA): 1 shift per day
- Fast Flow Ward: 08:00-13:00 (1.25 PA): 1 shift per day
Administrative duties are allocated 0.2 PA per 1 PA clinical duty.
Weekend commitments are long shifts on AECU Saturday and Sunday (08:00-20:30) every six weeks (equivalent to 1.4 PA per week).
Supporting Professional Activities are assigned to 1.5 SPA core SPA for continuous professional development. A remaining 0.5—1 SPA can be awarded for work in areas of governance, clinical supervision, departmental development projects, etc.

- Clinical sessions: as per table below (4.6 PA)
- Weekend duties (1.4 PA: as specified above)
- Admin time: 1 PA (15% of 6.0)
- SPA: 2.5 SPAs (1.5 SPA for Core SPA + 0.5 SPA for mortality lead + 0.5 SPA for educational supervision – at flexible times)

Training

- Minimum of 4 years of post MRCP training all of which include participation in acute medical rota work
- Attendance at appropriate professional meetings and courses

- Specialist experience in one or more areas of practice to take a lead role in that area
Experience

- Ability to assess and deal with medical cases in the resuscitation department

- Evidence of undergraduate and postgraduate teaching and commitment to these within the Trust
