Job overview

The successful candidate will work within the Frailty Team in the fast-paced Emergency Department and Acute Assessment Unit with opportunities to work on the South-Central Ambulance Falls/Frailty Car project. Frailty Specialists are the role models of exceptional, appropriate and compassionate care of older people living with frailty.

The post is open to all health professions who can demonstrate the skills to assess a patient’s physical, cognitive and social ability to remain at home or put plans in place to meet any acute or long term needs.

Main duties of the job

The frailty team work very closely with our community partners and are actively involved in the Hampshire Together programme. We expect that this role will work flexibly across hospital, community and ambulance services in line with the evolving integrated care strategy.

What we look for:

  • A passion for older adults
  • An understanding of the unique challenges someone living with frailty faces, especially when acutely unwell
  • Someone who values working as part of a close virtual multidisciplinary team
  • Excellent communication skills
  • Previous experience with older adults or those with complex needs
  • Advanced assessment and care planning skills using a holistic approach
  • A willingness and ability to work “hands-on” in all aspects of patient assessment and care
  • Completed or looking to complete post-graduate education related to Frailty

Working for our organisation

Our vision is to provide outstanding care for every patient. Patient care is at the heart of what we do at our three sites: Basingstoke and North Hampshire Hospital, Royal Hampshire County Hospital in Winchester and Andover War Memorial Hospital. Hampshire Hospitals NHS Foundation Trust provides medical and surgical services to a population of approximately 600,000 people across Hampshire and parts of West Berkshire.  Our cultural ambition is to have a culture that places people at the heart of all we do, where we all belong, and where learning, improvement and excellence thrive.

We provide specialist services to people across the UK and internationally. We are one of only two centres in the UK treating pseudomyxoma peritonei (a rare form of abdominal cancer) and we are leaders in the field of tertiary liver cancer and colorectal cancer.
The trust employs more than 9,000 staff and has a turnover of over £500 million a year. As a Foundation Trust, we are directly accountable to our members through the governors. The Council of Governors represent the interests of their constituencies and influence the future plans of the Foundation Trust.
