Job overview

The urgent and intermediate care team are pleased to be offering these posts to join our Care at Home Team, providing patient centred care to individuals in the community, alongside community nurses and allied health professionals. We are looking for passionate and experienced HCSWs to join our Pembrokeshire Team to support patients to stay in their own home.

Main duties of the job

In this varied and exciting role, you will be providing personal care to support patients in their own home. This can be for patients waiting for long term care, reablement and rehabilitation, continuing health care, or as part of their end of life / palliative care plan.

The post holder will be required to deliver a high standard of person centred care in accordance with Care and Treatment Plans as directed by the registered nurse. Excellent interpersonal and communication skills are essential as is the ability to work autonomously and with colleagues.

You will be working in the Community but have a contractual base at either South Pembrokeshire Hospital or Withybush General Hospital. The service operates 7 days a week between 0730 – 2200 and staff shifts are within these hours. You will be working in patients’ homes across the county and the ability to travel to locations in a timely manner is essential.

48 general practices (six of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;

Numerous locations providing mental health and learning disabilities services;

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
