Main duties of the job

We are looking to recruit experience, highly skilled and motivated nurses to join our Primary Care Team. Our aim is to deliver a wide range of healthcare interventions to improve our patient's overall health and well being. You will be a qualified Registered Adult Nurse (RGN) with experience in Prison Nursing along with a real passion for developing your clinical skills in an ever-changing environment. You will have a flexible approach to working within a team that encourages new ideas and innovations to improve the service. If you are self-motivated and able to work autonomously as well as showing plenty of initiative when it comes to handling a wide range of situations, then this could be the right role for you!

Qualifications / training

Essential criteria
  • Registered nurse adults tor & Assessor award or equivalent
  • Evidence of continuing Professional Development Caseload management experience

Experience

  • Significant post registration experience
  • Evidence of multi disciplinary team working

Skills/knowledge

Essential criteria
  • Sound knowledge of Palliative care and long term conditions management Leadership skills and ability to work in a supervisory capacity
  • Evidence of time and resource management
  • Evidence of assessment skills and individualized care planning
  • Custodial experience Knowledge of current issues and legislative changes affecting NHS, Health & Social care, particularly those pertinent to the primary care setting
  • Ability to teach patients and staff across all areas of care & practice
  • Ability to undertake clinical audit and demonstrate sound knowledge of standard setting and audit process

Skills/knowledge

Essential criteria
  • Ability to demonstrate individual responsibility in respect of Health & Safety
  • Ability to demonstrate written skills that are relevant ,accurate concise and legible
  • To have basic I.T. knowledge and inputting skills
  • Awareness of the needs of ethnic minority patients/carers, within the framework of valuing equality and diversity
  • Ability to work in a supervisory capacity
  • Demonstrate application of appropriate guidelines and protocols within clinical practice
