An exciting opportunity to join the community Heart Failure Service delivering care to the adult population within the South West region of Bristol, North Somerset and South Gloucestershire (BNSSG)

If you are looking for a career change or to develop your skills and expertise in a specialist area, this is a great opportunity to join our community Heart Failure service as a Heart Failure Specialist Nurse.

The successful candidate is expected to be highly motivated, dedicated with a passion for excellence in patient care, along with excellent communication and organisational skills.

This post may suit a clinician with previous cardiology experience or an experienced clinician with skills that are transferable to this position. There will be opportunities to support your own personal development, to help you gain the knowledge and skills to succeed.

You will see patients in community-based clinics and in their own homes and therefore a full UK driving licence is essential or ability to be able to travel independently to fulfil the role.

There will be an element of working across BNSSG to meet service delivery, but we look to deliver clinical caseload elements of the service within geographical locations with bases currently within BNSSG.

The post holder will manage a caseload of patients with complex Heart Failure, using evidence-based practice and patient centered goals to assess, plan, implement and evaluate interventions.

To provide expert support, advice, and education to raise awareness of the needs of patients with Chronic Heart Failure

To undertake all aspects of clinical duties as an autonomous practitioner with access to advice and support from senior colleagues

The ideal candidate will have a strong team work ethic; excellent communication skills; a committed professional approach to working with both patients and the team; a strong sense of accountability, accompanied by an ability to work under pressure at times.

  • Evidence of cardiology experience.
  • Able to evidence continuing professional development and take responsibility for your ongoing development, ensuring professional standards are maintained
  • Evidence excellent communication, negotiation and influencing skills
  • Demonstrable effective clinical reasoning & decision-making skills
  • Demonstrate ECG interpretation skills.
  • Able to work autonomously
  • Able to demonstrate an understanding of the implications of cultural differences within the health care setting.
  • Ability to travel throughout service delivery geographical area to meet the services needs

  • Previous experience of working in a community/ primary care setting
  • Experience of teaching multidisciplinary groups
  • Experience of undertaking health promotion activities with patients and carers.
  • Experience and knowledge of principles and practice of palliative care
  • Knowledge of shared decision making and goal setting. Ability to implement this within patient care
  • Be able to undertake comprehensive specialist assessments of patients including those who may have complex presentations or multiple pathologies
